how do you make a high quality iron on patch?

what do i need to know on how to make a patch for a shirt to be ironed on?
-high quality

I have seen iron on transfer paper for your printer at office supply stores. Never tried them so I am not sure if the are high quality. If you are using a fabric patch, maye fusible interfacing would work. I am not too sure about that though.
